Type Multi-ballpoint pen with a stylus
Product Name Matrix Multi-pen.
Manufacturer and Year Cross - they made this pen fairly recently. They no longer make the pen, but they still make refills which are for sale on the Cross website.
Length The total pen is 6", the ballpoint/stylus that detaches is 3-1/4".
Filling System The main multipen part takes Cross's tiny Matrix ballpoint pen refills, and a small packet of black ones is included in the box, and uses two at a time, so you can extend a different color depending on which direction you twist the tip. The other end is a torpedo-shaped piece, which has a stylus on one end and a ballpoint on the other. It can be pulled out to use the ballpoint, which is a special refil, one of the 7 you see in the photo below on the right side of the box. The options of refils are 6 different colors, or a yellow highlighter. It also comes with an extra stylus/optional ballpoint detachable piece. It looks like it had another which was a fountain pen piece, and there is what looks like a fountain pen ink cartridge with the other refills, but the fountain pen piece is not in the set.

Condition It's in nearly perfect condition with hardly any signs of use. The green refill in the box seems to have opened up slightly, and released some of it's suspending fluid, which got on part of the box. Because of this the green refill may not work, or its ink may not flow smoothly, though we have not tested it. The pen itself shows almost no signs of use, and seems to be working perfectly.
